A: Rocky Mountain Brain Injury Services offers adult day services, behavioral management, day treatment,
mental health counseling, non-medical transportation, substance abuse counseling. We offer Colorado’s
only post-hospital residential program under the Colorado Home and Community Based Services Waiver-a
long term care Medicaid program
A: Rocky Mountain PACE offers services to the following area codes: 80903, 80904, 80905, 80906, 80907,
80909, 80910, 80914, 80915, 80916, 80917, 80919, 80922 and 80923. Please check back soon for
additions to our PACE service area.
A: Financial applications can be made at the Department of Human Services in your county of residence.
For
El Paso County residents, please apply at 105 N. Spruce in Colorado Springs or
740 Hwy 24 in
Woodland Park if you reside in Teller County.
A: Your family member may be eligible for services under one of our long term care programs. If you
reside in El Paso or Teller County, contact Rocky Mountain Options for Long Term Care at (719) 457-0660
to speak to someone in our intake department. One of our case managers will help determine your
family member’s functional eligibility for community based services. These programs are covered under
Colorado Medicaid—financial eligibility is determined through the Department of Human Services.
