Rocky Mountain’s Program of All-Inclusive Care for the Elderly (PACE)
This program is known as the gold standard of care for the frail elderly.
PACE delivers the entire continuum of medical and supportive services to seniors living in El Paso county with chronic care needs, while promoting and maintaining their independence in their homes for as long as possible.
Rocky Mountain PACE will organize and alleviate the confusion created by traditional medical and support services that act independently of one another. Rocky Mountain PACE participants are regularly assessed by an interdisciplinary team (IDT) who work together to promote the health of each individual. This team includes a PACE physician, nurse, social work, physical and occupational therapies, dietician, mental health therapist, transportation as well as other staff necessary to the well-being of our participants.
Rocky Mountain PACE services include:
- Adult Day Center with medical and support staff
- Home health and personal care
- All necessary prescription drugs
- Medical specialists such as audiology, dentistry and podiatry
- Respite care
- Hospital and nursing home care when necessary.
